Privacy Policy: I agree to the Privacy Policy and consent to Pure Health & Wellness Clinic collecting and storing my personal and health information for the purpose of providing healthcare services.*

Cancellation Policy: I acknowledge that a fee of 50% of the consultation cost will be charged if less than 24 hours cancellation notice is provided.*

Consent to Treatment: I voluntarily consent to naturopathic treatment, including but not limited to lifestyle and dietary advice, herbal and nutritional supplementation, homeopathic remedies, manual therapies and any other treatments recommended by my practitioner. I understand that naturopathic treatment is complementary to conventional medicine and is not a substitute for emergency or primary medical care. I have been advised to continue seeing my GP and to call 000 in an emergency.*

Financial Consent: I understand that consultation fees are payable at the time of the appointment. Cancellation within 24 hours of the appointment may incur a fee. Health fund rebates are processed where applicable but any gap is my responsibility.*
